FLORHAM PARK, N.J., Oct. 4 /PRNewswire/ -- Shionogi Inc., the U.S.-based group company of Shionogi & Co., Ltd., today announced the U.S. Food and Drug Administration approval of the non-stimulant medication Kapvay (clonidine hydrochloride) extended-release tablets, an extended-release oral formulation for the treatment of Attention Deficit Hyperactivity Disorder (ADHD) in children and adolescents ages 6-17 years. Kapvay is the only formulation of clonidine hydrochloride approved by the FDA for the treatment of ADHD, and is the first and only FDA-approved ADHD treatment indicated for use as add-on therapy to stimulant medication. Kapvay can also be used as monotherapy when treating ADHD.
An oral, non-stimulant, twice-daily therapy, Kapvay is a centrally acting alpha2-adrenergic receptor agonist. While the mechanism of action of alpha2 agonists in ADHD is not known, it is believed to involve the pre-frontal cortex (PFC) of the brain. Studies suggest that the PFC regulates attention and plays a critical role in impulse control, working memory and executive function.
"The FDA approval of Kapvay represents an exciting milestone in the field of ADHD," said Donald C. Manning, MD, PhD, Chief Medical Officer of Shionogi Inc. "The extended-release formulation of Kapvay minimizes the peaks and troughs in blood levels, thereby decreasing overactivation of the alpha receptors in the brain and periphery. "ADHD is a complex disorder that requires individualized treatment. While there are prescription treatment options available, many ADHD patients on stimulants do not achieve adequate control of symptoms," explained Rakesh Jain, MD, MPH, Director of Psychiatric Drug Research for R/D Clinical Research at Lake Jackson, Texas, and an investigator in the clinical trials. "Kapvay, when added to a stimulant, addresses an unmet need, and improves ADHD symptoms beyond what is achieved by stimulants alone. This is a significant step forward for the treatment of ADHD to have an approved product for add-on therapy in our treatment armamentarium."
ADHD is a neurobehavioral disorder that occurs in childhood and may continue into adolescence and adulthood, which affects more than 4.5 million children ages 3-17 in the U.S. alone. Approximately 3-7 percent of U.S. school-aged children are believed to suffer from this disorder. Symptoms include difficulty staying focused and paying attention, difficulty controlling behavior, and hyperactivity/over-activity.
This approval is based on two Phase III studies, which demonstrated efficacy at 5-weeks that children and adolescents (6-17 years) with ADHD treated with Kapvay experienced statistically significant improvements in core symptoms of ADHD – inattention, hyperactivity and impulsivity. The most common and drug related adverse reactions (incident at least 5% and twice the rate of placebo) included somnolence, fatigue, upper respiratory tract infection (cough, rhinitis, sneezing), irritability throat pain (sore throat), insomnia, nightmares, emotional disorder, constipation, nasal congestion, increased body temperature, drug mouth and ear pain. Maintenance efficacy has not been systemically evaluated and patients who are continued on longer-term treatment require periodic reassessment.
About Kapvay
Administered orally, clonidine hydrochloride exerts its pharmacological effects as a centrally acting alpha2-adrenoceptor agonist. The formulation in Kapvay is designed to delay the absorption of active drug in order to decrease peak to trough plasma concentration differences.
Important Safety Information
Kapvay should not be used in patients with known hypersensitivity to clonidine.
Kapvay has not been studied in children with ADHD less than 6-years old.
Use Kapvay with caution in patients at risk for hypotension, bradycardia and heart block. Measure heart rate and blood pressure prior to initiation of therapy, following dose increases, and periodically while on therapy. Advise patients to avoid becoming dehydrated or overheated.
Somnolence and sedation have been observed with Kapvay. Consider the potential for additive sedative effects with CNS depressant drugs. Caution patients against operating heavy equipment or driving until they know how they respond to Kapvay.
Patients should be instructed not to discontinue Kapvay therapy without consulting their physician due to the potential risk of withdrawal effects. Kapvay should be discontinued slowly in decrements of no more than 0.1 mg every 3 to 7 days.
In patients who have developed localized contact sensitization or other allergic reaction to clonidine in a transdermal system, substitution of oral clonidine hydrochloride therapy may be associated with the development of a generalized skin rash, urticaria or angioedema.
Use in patients with vascular disease, cardiac conduction disease, or chronic renal failure: Monitor carefully and uptitrate slowly.
Clonidine may potentiate the CNS-depressive effects of alcohol, barbiturates or other sedating drugs.
Caution is warranted in patients receiving clonidine concomitantly with agents known to affect sinus node function or AV nodal conduction (e.g., digitalis, calcium channel blockers and beta-blockers) due to a potential for additive effects such as bradycardia and AV block.
Do not use Kapvay concomitantly with other products containing clonidine.

A War For Getting Maximum Profits
HAYWARD, Calif. -- Impax Laboratories Inc. said Monday it is being sued by a unit of Johnson & Johnson, accused of infringing on a patent supporting the attention deficit hyperactivity drug Concerta.
Impax and its partner Teva Pharmaceutical Industries Ltd. are trying to get regulatory approval to sell low-cost versions of extended-release 54-milligram Concerta tablets. Impax said it is being sued by Johnson & Johnson’s Alza Corp. unit.
Earlier this month, Johnson & Johnson reached a deal that will allow Watson Pharmaceuticals Inc. to sell a generic version of Concerta. Watson can start selling its version on May 1. Johnson & Johnson will supply Watson with the drug and receive a share of net sales until the deal ends in 2014. Watson and J&J had been involved in a patent dispute. A court ruled that the patent at the center of the lawsuit was not valid.
After that ruling, Impax amended its filing with the FDA.
Impax said U.S. sales of Concerta 54-milligram tablets totaled $365 million over the 12 months ended Sept. 30.
Shares of Impax Labs fell 38 cents, or 2 percent, to $18.77 in afternoon trading. Teva shares rose 26 cents to $50.94 and shares of Johnson & Johnson lost 38 cents to $61.92.

The word paranoia comes from the Greek "παράνοια" (paranoia), "madness"[1] and that from "παρά" (para), "beside, by"[2] + "νόος" (noos), "mind"[3]. The term was used to describe a mental illness in which a delusional belief is the sole or most prominent feature. In original attempt at classifying different forms of mental illness, Kraepelin used the term pure paranoia to describe a condition where a delusion was present, but without any apparent deterioration in intellectual abilities and without any of the other features of dementia praecox, the condition later renamed "schizophrenia". Notably, in his definition, the belief does not have to be persecutory to be classified as paranoid, so any number of delusional beliefs can be classified as paranoia. For example, a person who has the sole delusional belief that he is an important religious figure would be classified by Kraepelin as having 'pure paranoia'. According to Phelan, M. Padraig, W. THE WEEK posted a pretty good article called “ADHD’s Rapid Rise: 5 Theories.” We say it’s pretty good because though several of their theories may play some part in why so many kids are diagnosed ADHD, they never quite nail the answer. So we did.
Adding to their 5 points of various theories, we present you with point number 6: The actual answer:
6 . Psychiatrists got together and decided to pathologize normal childhood behavior into a mental disorder and call it ADHD. They created a checklist of behaviors, took a vote on it, and voilà! A whole new client base was born – kids. With the help of billions in Pharma funds spent on shrinks to promote ADHD in journals, on TV and in press, glossy ads in magazines, slick lobbyists to “educate” members of Congress about it, and the creation of Pharma front groups such as Children and Adults with Attention Deficit Disorder (CHADD) to infiltrate schools endorsing the so-called disease —an epidemic of “mentally ill” children was born. And that’s the real reason for the “rapid rise” in kids diagnosed ADHD and put on drugs. Drugs the U.S. Drug Enforcement Administration (DEA) categorizes in the same class of highly addictive substances as cocaine and morphine—drugs such as Ritalin, Adderall, Concerta — documented by the US FDA to cause hallucinations, mania, heart attack, stroke, sudden death to name but a few. And it all starts with one simple thing: The Diagnosis. (We challenge anyone to find a kid that would not fit some, if not all of psychiatry’s criteria for a “mentally ill” child they call ADHD)

Fails to give close attention to details or makes careless mistakes in schoolwork, work, or other activities.
Has difficulty sustaining attention in tasks or play.
Does not seem to listen when spoken to directly.
Does not follow through on instructions and fails to finish schoolwork, chores, or duties in the workplace (not due to oppositional behavior or failure to understand instructions).
Has difficulty organizing tasks and activities.
Avoids, dislikes, or is reluctant to engage in tasks that require sustained mental effort (such as schoolwork or homework).
Loses things necessary for tasks or activities (e.g., toys, school assignments, pencils, books, or tools).
Easily distracted by extraneous stimuli.
Forgetful in daily activities.
Fidgets with hands or feet or squirms in seat.
Leaves seat in classroom or in other situations in which remaining seated is expected.
Runs about or climbs excessively in situations in which it is inappropriate (in adolescents or adults, may be limited to subjective feelings of restlessness).
Has difficulty playing or engaging in leisure activities quietly.
Appears “on the go” or acts as if “driven by a motor.”
Talks excessively.

That’s a shocking 22 percent jump over 2003 figures — representing an additional 1 million children — and the increase was seen in all races, income levels, and areas of the U.S., with the exception of the West. What’s behind the rise? Here are 5 theories:
1. Doctors are doing a better job of diagnosing ADHD
Improvements in screening programs and greater awareness of the disorder among parents and doctors have helped identify more cases, says CDC epidemiologist Susanna Visser, , the report’s lead author. “We have become much more sensitive to behavioral differences,” agrees Dr. Jeffrey Brosco, an ADHD expert at the University of Miami. But that doesn’t mean doctors can say “whether kids in the 1970s are really different from kids in the ’90s or the 2000s.”
2. Demographics
The increases were more significant in certain demographic groups, note Scott Hensley at NPR. “The biggest jumps were seen in children between 15 and 17 and among Hispanic or multiracial children.” The jump in Hispanic ADHD cases likely reflects “greater cultural acceptance of the disorder.” Mysteriously, increases were particularly significant in 12 states, says Ray Hainer at CNN. North Carolina, for example saw a 63 percent spike in cases, with 15.6 percent of its kids diagnosed with ADHD.
3. Big Pharma is pushing the cure
Of the 5.4 million kids diagnosed with ADHD, the CDC reports, 2.7 million are taking medication for the condition. You have to question “the role of pharmacological companies in all of this,” says University of Kentucky psychiatrist John D. Ranseen. “It is very much in their interest to increase the diagnosis and treatment of this condition.” That alone should “give the mental health field pause.”
4. Blame our lousy diet
Nobody really knows what causes ADHD, says David Knowles in AOL News, but “one recent study suggested a correlation with a diet high in processed and fried foods.” Intriguingly, new research also ties ADHD to obesity in adulthood, says Healther Turgeon in Strollerderby. There’s no proof — yet — that one causes the other, but “the two are correlated.”
5. The real spike is in “paranoid” parents
“Are kids really that messed up?” asks Tom Henderson in ParentDish. “Or are parents becoming a bunch of second-party psychological hypochondriacs?” Remember, these million extra ADHD cases are “parent-reported diagnoses,” and today’s parents have been known to be “all too eager to control normal childhood restlessness and general weirdness by bombing kids with Ritalin.” Because, after all, “children often have the attention spans of, uh, children.”
This is the truth we are creating a pool of dangerous criminals.

Many of us -apart from blind people- born and live with functional onboard cameras and hard disc memory capacity. When you get older, both will betray you. Currently, we might not be able to replicate what nature can do as a living organism, although at least we know the logic, limits and major roles of the components. We can predict that it's not an "essence" going inside of our brains, but it is a repeatable technology. Set of representations and interpretations: Cable carries electricity, but this electricity was already sent by a sensor in certain frequency and voltage, so a different electronic switches start to interpret light, colour, depth and produce a photography.
Nature does the similar thing, although it uses organic material instead of slicon. It may be doing it in a more sophisticated fashion than our non-replicating digital gadgets -and their 0-1 logic-; but it is a fact that it can not produce a material copy of the image. When we exactly grasp the technology behind the brain, we would be able to see the imagination board of our brains, we would download it, share it, motion picture it, print it; and other way around journey as well: We could send images and other information to brain's mental board.
Only if we could crack the system information code... but my opinion is NO so we have to leave these areas which are not in our control.

We know that this answer is out there, and some clever people can explain its technology. We know its whole 150 years of history and we know how it has been evolved through generations, and how does it work in transistor level. Current knowledge of DNA does not allow us to get the absolute working mechanism of a living thing, we've just started to isolate the individual genes and make artificial copies of them. We are kind of harvesting the nature in DNA level, just as our ancestors had harvested the soil to get what they needed from nature.

MY LOST DREAM:My Grandfather Late Sh. Pandit Hamir Singh Was a Senior Congress Leader of Bathinda ,Mansa Falls Under Bathinda At That Time.During My Childhood Days When I Started Recognize My Near's & Dear's.I was Very Much
Attached & Inspired by the Man Famously Called Panditji by Everyone.My Grandfather Was at The Peak at That Time But His Feet Always remained On Earth.He was a Gem Of politics.I See His Popularity In The Malwa Region N Number Of Persons Come Everyday To Meet him And to Share Their Problems.The Man Was Always Ready to Sort Every Matter Came Under His Consideration.He Never Demands Any Favours From The Party Even Opposition Parties Respect Him & Take Opinion for Some Issues From Panditji.That Was The Period When Punjab Was Badly Affected by Terrorists and One Day It Happens to Us Panditji Was Killed by Terrorists While Due to back from Bathinda After Party Meeting.Everybody was Stunned by The News Coming but Most Affected Was Me, Something Breaks Inside Me and it Was Cleared Within a Year That a Good Boy Was Completely Transformed Into Drug Addict,full of Rage Fight with Anyone .Even I Tried To Shot at a Person Who Was Responsible For my Grandfather's Death,However He Managed to Escape Due To My Parents .He Was Very Close To Late Sh. Rajjeev Gandhi he Told Me Once That Rajiv Ji Always Said Panditji Mein Aapse Pehle Maronga but Here Also Panditji Win.I See Late Beant Singh And Other Senior Leaders First Time and Occasion Was His Cremation.Panditji Left Me 17yrs before alone nobody understands me like he was.I Was Failed to Walk on His Foot Steps .Never Recovered From That Tragedy.

Pls Grandpaa Forgive Me , Today I Need You Badly, Miss You Badly . I have No Other Dream To Follow becoz They Also Flies After Your Departure.No Tears Into My Eyes But My Heart Is Overloaded Now With The Sufferings, I Never Shared With Anybody & A Sudden Explosion Is Dangerous for The Family & to The Society.I am Living For Other's Becoz I have nothing To Live For.
